OL97: Custom Forms are No Longer Listed on the Compose Menu

When using Microsoft Outlook 97 version 8.03.4629 and applying the
following QFE files, custom forms are no longer visible on the Compose
menu.

Install the Microsoft Office SR-2 patch.
Microsoft has confirmed this to be a problem in Microsoft Outlook 97,
8.03.4629 after applying the above QFE files. This problem was corrected
in Microsoft Office 97 Service Release 2 (SR-2).
